New Forming ACRM Transitional Care Group

 

The new Transitional Care Group aims to provide a unique opportunity to illuminate important facets of rehabilitative services and outcomes through integrated and strategic support, collaboration, and education.

 

MISSION

To be the leading organization to engage key stakeholders (e.g., scientists, clinicians, people with lived experiences, community organizations, practitioners, administrators, policymakers) to drive the science on care transitions related to rehabilitation, informing dissemination into practice.

 

VISION

To improve science, practice, policy, and access in care transitions related to people with rehabilitation needs.

2025 GOALS

  • Science Goals: 1) To develop common data elements specific to care transitions; 2) To collaboratively conduct a scoping review with a multi-faceted care transitions focus for rehabilitation populations to establish the current state of knowledge in care transitions, identifying gaps to inform next steps
  • Practice Goals: 1) To develop one or more I/EPs (information education pages) specific to care transitions to be published on Archives [definition: brief (2-page) sheets that provide consumer-friendly information on topics relevant to rehabilitation medicine]; 2) To begin planning for development of best practices/clinical guidelines development
  • Policy Goal: 1) To engage and inform policy and decision makers on the opportunities to improve policy and payment reform related to care transitions for rehab populations
  • Access Goal: 1) To engage practitioners, persons with lived experience, and community groups specific to care transitions
