Course Description

This workshop will present a state-of-the art measurement system for assessment of neurological and behavioralfunction that is designed to bring a standard of measurement to practice and research –the NIH Toolbox®. Available in both English and Spanish, the recently launched iPad version of the NIH Toolbox is a highly portable and easy-to-use set of measures that can be implemented across diverse study designs and settings. Rehabilitation professionals are frequently required to assess a wide variety of symptoms, function and abilities in research and practice settings. Unfortunately, the sea of available measures for these purposes is vast. It includes measures of widely varying quality, many of which are costly to use and burdensome in terms of length and high reading levels. The multiplicity of measures used across studies limits the degree to which result across studies can be compared. In order to help standardize measurement of important functional areas, the National Institutes of Health (NIH) supported development of the NIH Toolbox® for Assessment of Neurological and Behavioral Function. The NIH Toolbox is a set of brief, reliable, minimally burdensome measures of cognitive, motor, emotional and sensory function for use from ages 3-85 years (Gershon, et al. 2010). Developed using both classical and modern test theory and methods, the NIH Toolbox provides scores that are comparable across a broad range of age and ability levels, making it uniquely suited for use in lifespan assessment and evaluation of disorders covering an extensive spectrum of severity. Comprised of four 30-minute batteries, the core suite of measures can be administered in two hours or less. The batteries assess the following constructs: Cognition Battery -Executive Function, Attention, Episodic Memory, Language, Processing Speed and Working Memory; Motor Battery-Dexterity, Grip Strength, Standing Balance, Gait Speed, and Endurance; Emotion Battery (self-and parent-report) -Psychological Well-Being, Social Relationships, Stress and Self-Efficacy; Sensory Battery-Audition, Visual Acuity, Olfaction and Taste. All measures were evaluated for accessibility and cultural and ethnic applicability, and have demonstrated good reliability and validity. The NIH Toolbox was normed using a sample of 4,859 participants (English and Spanish speaking), ages 3-85, representative of the U.S. population based on gender, race/ethnicity, and socioeconomic status. A variety of normative scores are available. Through this interactive course, we aim to equip attendees with the skills needed to select, administer, score, and interpret NIH Toolbox measures. We will cover the science behind Toolbox development, including an introduction to item response theory (IRT) and computer adaptive tests (CATs). We will describe each battery, its implementation in the NIH Toolbox iPad app, and available score reporting options. We will discuss NIH Toolbox accessibility and use with people with disabilities. Participants will then be given hands-on experience using the NIH Toolbox iPad app on iPads provided by us. Following an overview of recent findings on NIH Toolbox validation and use in clinical populations, the workshop will conclude with a discussion of questions regarding clinical and research applications raised by course participants.

 

LEARNING OBJECTIVES

Upon completion of the course, participants should be able to:

  1. Upon completion, participants will understand the history and development of the NIH Toolbox.
  2. Participants will be given a hands-on overview of how to select and administer NIH Toolbox measures.
  3. Participants will understand the different scoring options and how to select the most appropriate for their needs.
  4. Participants will learn about Computer Adaptive Testing and how this was incorporated into the NIH Toolbox.
