Course Description

Managing costs and delivery of the safest and most appropriate care throughout the continuum of the patient’s experience is essential in order to provide the highest quality of care in the most cost-efficient manner.  We will discuss various evidence-informed risk assessment tools to best evaluate patients for the most appropriate postacute setting and discuss our continuum of care pathway. Encompass Health Corp. currently owns 130 inpatient rehabilitation hospitals and is the fourth largest provider of home health and hospice services.  We will present our streamlined approach to evaluating and delivering postacute care to patients across the continuum, with the ultimate goal of allowing patients to achieve maximum functional improvements with the lowest risk of readmission to the acute care hospital.

 

LEARNING OBJECTIVES

Upon completion of the course, participants should be able to:

  1. The attendee will be able to name and utilize two risk assessment tools to evaluate patients for appropriate post-acute placement.
  2. The attendee will be able to define the roles of the care navigator and care transition coordinator and discuss how they would implement in their setting.
  3. The attendee will be able to identify three examples of clinical collaboration to avoid rehospitalization during the episode of care.
  4. The attendee will understand the role of predictive analytics in evaluating risk of readmission.
