Course Description

Chemotherapy-induced peripheral neuropathy (CIPN) presents as sensory pain, numbness, or paresthesia. CIPN affects activities of daily living (ADL), work, play, leisure and social participation. It impacts overall function and has a profound negative effect on people’s quality of life.
Certain chemotherapy agents are known to cause CIPN but are necessary for best medical management. At least thirty-to-forty percent of people receiving chemotherapy experience CIPN and this figure is reported by authorities to be an underestimation. As more aggressive pharmacological agents are developed and survival rates increase in the future, this number is expected to grow. Some patients have to discontinue their medication or reduce dosage due to the severity of the neuropathy. Such difficult choices impact quality of life and may also shorten lifespans.
In the oncology literature, there are no descriptions or acknowledgments of the value of rehabilitation specifically targeting the sensorimotor system for patients with CIPN. Articles in oncology journals refer to rehabilitation but appear to define rehabilitation as medication or alternative/integrative strategies for which the evidence is generally weak. Complicating matters, oncologists are not typically in the habit of referring their patients with CIPN for rehabilitation with the goal of sensory symptom reduction or resolution. The few therapy programs that are described tend to emphasize compensatory and adaptive strategies rather than more specific interventions aimed at promoting health and function of the nervous system.
This instructional course identifies a rehabilitation program based on interventions which have been extrapolated from evidence-based treatments used to facilitate sensory recovery. The theoretical rational supporting this program will be explained. Strategies promoting the development and marketing of this program will also be offered. Update of a current research project will be previewed and recommendations for future related research will be made. Participants will learn about upper extremity (UE) and lower extremity (LE) motions and activities targeting the neuromuscular system. In addition to demonstration, participants will exercise along with and while watching video clip samples of home exercise programs in order to maximize learning and to promote audience participation. Resources for home program instructions and ideas will be provided.

 

LEARNING OBJECTIVES

Upon completion of the course, participants should be able to:

  1. Upon completion, participants will be able to describe chemotherapy-induced peripheral neuropathy (CIPN) and its impact on life roles and overall quality of life
  2. Upon completion, participants will be able to distinguish between sensory pain and sensory impairment and will apply this distinction to their clinical reasoning in treating CIPN
  3. Upon completion, participants will be able to demonstrate 3 evidence-based rehabilitation interventions for sensory impairment or sensory pain due to CIPN
  4. Upon completion, participants will be able to provide a home program for symptom management that includes a client’s family member or significant other as appropriate
