Course Description

Developments in item response theory (IRT) and technology over the past two decades allow for precise, targeted, and efficient assessment of rehabilitation outcomes. Scores from IRT-developed outcomes measures allow clinicians to track patient improvement, set realistic and data-driven goals, and develop better discharge plans. Traditional sum-scored measures, on the other hand, are saddled with many disadvantages. Sum scoring cannot accout for unequal intervals and varying task difficulty on rating scales (see Wright & Linacre, 1989; Merbitz, Morris, & Grip, 1989), nor are they appropriate when a measure assesses multiple correlated patient abilities simultaneously. IRT is useful in such complex assessment scenarios. In this course, I will demonstrate how to apply recent developments in IRT to a clinical outcomes measure. Attendees will learn how to determine the number of distinct traits a measure assesses, build different models that can further their understanding of how those traits may be related, evaluate item properties such as discrimination and difficulty, and estimate a variety of IRT models appropriate for different applications. Example code and syntax for popular psychometric software will also be provided.

LEARNING OBJECTIVES

Upon completion of the course, participants should be able to:

  1. Describe fundamentals of item response theory (IRT)
  2. Describe different types of IRT models
  3. Describe applications of IRT models to clinical outcomes assessment
  4. Describe innovate use of IRT models in rehabilitation research and practice
