Course Description

In many instances, the processes and outcomes associated with care transitions for stroke survivors have room for improvement.The purpose of this four hour instructional course is to increase essential knowledge and skills required to optimize the effective care transitions and determination of the most appropriate outcomes of stroke survivors and their families. Quality measures, evidence-based transitions of care models and recommendations for practice will be discussed along with opportunities to apply this knowledge to participants’ practice settings and future research.

 

LEARNING OBJECTIVES

Upon completion of the course, participants should be able to:

  1. Describe quality measures and current performance nationally
  2. Identify the major factors affecting readmissions of stroke survivors
  3. Discuss opportunities to improve care transitions
  4. Review the evidence supporting stroke care transitions
  5. Discuss key program elements to promote successful transitions from acute care through to the community setting
